The subciliary approach involves a precise incision just below the lower lash line. Through this approach:
Excess skin is excised to correct sagging and crepey texture
Orbicularis oculi muscle (OOM) suspension is performed to reinforce lower eyelid support
Fat removal and/or fat repositioning is used to smooth under-eye bulging and improve the lid–cheek junction
Careful closure minimizes visible scarring

Transconjunctival Approach
The transconjunctival approach accesses the lower eyelid through an incision inside the eyelid, leaving no external skin incision. Through this internal route:

Fat repositioning can be performed to redistribute volume over tear troughs and hollow areas
Skin and muscle are preserved, maintaining natural eyelid support
